We know how that sounds. The moment most people hear the words “we need to reduce a little enamel,” they sit up straighter in the chair and start asking questions. Good. You should ask questions. And that is exactly why we are writing this, so by the time you finish reading, you will not just understand what IPR is, you will actually get why it is one of the smartest moves in your entire treatment plan.

Okay, So What Is IPR?
IPR stands for Interproximal Reduction. Strip away the jargon and here is what it actually means. A tiny, controlled amount of enamel is gently removed from the sides of certain teeth using a fine filing strip or a small disc. The amount we are talking about is so small it is almost hard to believe. Usually between 0.2 and 0.5 millimeters per contact point. That is thinner than a fingernail.
Here is a way to picture it. Imagine trying to rearrange a packed bookshelf without pulling any books out first. You just cannot do it. Your teeth are the same. When they are crowded or overlapping, there is simply no room for them to move into better positions without creating a little space first. IPR creates that space. Precisely, safely, and exactly where it is needed.
It does not hurt the way you might be imagining either. Your enamel has no nerve endings so the procedure itself feels more like a mild vibration than anything uncomfortable. Most patients are surprised by how quick and uneventful it actually is.
Why Your Treatment Plan Cannot Just Skip It
This is the part that really matters and we want you to understand it properly.
IPR is not something your orthodontist throws into your plan because they feel like it. Every single instance of IPR in your treatment is calculated based on your specific tooth sizes, the width of your jaw, and exactly how much movement your teeth need to make. Your 3D treatment plan maps out which teeth need it, how much, and at what stage it should happen.
So what happens when it gets skipped? A few things, and none of them are great. Your aligners can stop tracking correctly, meaning your teeth stall and stop moving the way the plan intended. You can end up with what are called black triangles, those small visible gaps that form at the gum line when teeth are not shaped to sit properly against each other after moving. And in many cases the final result simply does not match what was planned because the mechanics of the whole movement were compromised from day one.
Skipping IPR to avoid a slightly awkward five minute procedure is genuinely not worth it. You end up needing more refinement trays, more time, and more frustration trying to chase a result that was never going to land without the groundwork being done properly. One More Thing Worth Knowing
A lot of patients worry that filing enamel will somehow weaken their teeth long term. It is a completely fair concern and we respect you for asking it.
The enamel on the contact points between your teeth is typically between 1.5 and 2.5 millimeters thick. The amount removed during IPR sits well within the safe range and has no meaningful impact on the strength or health of your teeth when it is done correctly. The key phrase there is done correctly. This is why the quality and precision of your treatment plan matters just as much as the quality of your aligners.
